Summary
This parent interview questionnaire was designed for use in obtaining a detailed case history for children referred to special education. It is used with parents whose home language is Spanish, to help in the assessment of children from birth through eight years of age. Its purpose is to discover possible physical causes for problems observed in the school setting. Questions cover physical injuries, illnesses, defective hearing and other factors that could be responsible for school-related problems. Questions are given in both English and Spanish. The interview is not designed to be used as the sole instrument for determining special program placement. It should be used in conjunction with other appropriate measures.
